EMPLOYEE WAS MAKING ROUTINE WALK AROUND OBSERVATION OF WASH PLANT AREA WHILE PLANT WAS RUNNING, WHEN A SHOE CAME LOOSE ON THE INSIDE OF A SPOKANE CRUSHER. EMPLOYEE WAS SHUTTING CRUSHER DOWN WH EN HE KNOCKED OFF FROM CRUSHER FROM THE BOUNCINGOF PLANT.
EE WAS REMOVING ROCK THAT HAD BOUNCED UNDER GRIZZLY BELT WHEN ROCK CAME LOOSE EE STRIANED LOWER BACK.

EE WAS MAKING REPAIRS ON SAND WASHING EQUIPMENT UPON COMPLETION OF TASK HE STEP DOWN FROM SAID PIECE AND LOST HIS FOOTING HE CAUGHT HIS WEIGHT ON THE INSIDE LEFT ELBOW AREA CAUSING A PULLED MU SCLE AND ALSO A BRUISE ON LOWER RIGHT SIDE RIB AREA.

AFTER MAKING ADJUSTMENT TO THE DRIVE BELTS OF A JAW CRUSHER, EE BEGAN TO NOTICE IRRITATION TO THE LEFT EYE WHICH PROGRESSIVELY GOT WORSE. HE WENT TO DR WHO REMOVED A FOREIGN OBJECT FROM HIS L EFT EYE. HE WAS EXAMINED BY DR NEXT DAY AND TOLDNOT TO RETURN TO DR UNLESS HE HAD PROBLEMS WHERE THE OBJECT CAME FROM OR HOW IT ENTERED THE EYE IS UNKNOWN.
