ROOF FALL OCCURRED AT THE FIRST INTERSECTION STA-1+08.25 AND FELL ABOVE THE 5FT BOLTS- 1 TO 2 FT. NO DAMAGE TO PERSONS OR EQUIPMENT. IT IS BELIEVED THAT THE CAUSE OF THE FALL WAS DUE TO THE C LOSE PROXIMITY OF THE HIGH WALL WHICH WAS HEAVILY SHOT DURING STRIPPING ALLOWING WATER TO COME THROUGH THE TOP TO CAUSE THE UNSTABLE CONDITION.
