VICTIM WAS OPERATING A TUGGER WHICH WAS ATTACHED TO A GROUT BOX, 1/2 FULL OF GROUT TO POSITION BOX FOR PERPENDICULAR LIFT BY THE QUARRY DERRICK. TUGGER PULLED OFF THE BASE-HOLD-DOWN PINS AND K NOCKED VICTIM OFF BENCH TO QUARRY FLOOR 18 FT BELOW. SAFETY LINES WERE PROVIDED, HOWEVER, THE VICTIM WAS NOT TIED OFF.

MR HALE WAS HELPING TO CHAIN AND TURN A PIECE OD GRANITE AND WHEN THE STONE WAS LIFTED A CHUNK BROKE OFF THE LARGE PIECE OF GRANITE AND FELL STRIKING MR. HALE IN THE CHEST. M HALE WAS ABOVE TH E QUARRY (NOT IN THE QUARRY) WHEN THIS ACCIDENT HAPPENED. HE WAS IN THE YARD WHERE THE STONE IS PLACED AFTER BEING TAKEN OUT OF THE QUARRY ITSELF.
