TUGGER PULLED OFF THE BASE-HOLD-DOWN PINS AND K NOCKED VICTIM OFF BENCH TO QUARRY FLOOR 18 FT BELOW. SAFETY LINES WERE PROVIDED, HOWEVER, THE VICTIM WAS NOT TIED OFF.
