AT THIS TIME THE CAUSE OF THE ACCIDENT IS UNKNOWN. ON SAT. 10-30-99 HE WAS INSTALLING A FRONT TIRE ON A STONE SLINGER IN OUR SHOP. WE KNOW THAT THE TIRES WERE MOUNTED ON THE RIMS AND AIRED UP. WE ALSO KNOW THAT ONE TIRE WAS ON THE FRONT AXLE OF THE TRUCK. AFTER THAT, WE DO NOT KNOW WHAT OCCURRED. THE EXPLOSION WAS HEARD BY 2 PEOPLE IN THE DISPATCH OFFICE WHO RAN OUT AND FOUND HIM A

Customer was loading material , wanted to dump some off, removed tailgate pin and ran, but tailgate hit the back of his calf causing a gash in his leg. This is a customer, not an employee. No other information in known.
EE was helping to remove a piece of plywood from the bed of the pup trailer. It cames loose and landed on top of his left foot. Possible broken bone in top of foot (swelling).

WHILE ASSISTING AN OUTSICDE VENDOR TO INSTALL A BUCKET SCALE ON A LOADER, WHILE UNDER VENDORS DIRECTION AND CONTROL HE WAS INSTRUCTED TO REMOVE A HYDRAULIC LINE. UPON DOING SO HE WAS SPRAYED W IHT HYDRAULIC OIL TRIED TO GET OUT OF THE WAY AND IN DOING SO BECAME TRAPPED UNDER THE BUCKET AS IT SETTLED INTO THE GROUND.
