IR DMM2 drill, CO# 114. Hydraulic line burst while drill was in operation causing fluid to spray on engine which created an instantaneous fire. Drill operator immediately exited drill with no injuries. Fire was contained by site personal using fire extinguishers and site water truck.
The ground area around a collapsed hole gave way; employee fell backwards and into the hole. Diagnoses: cervical strain; contusion, shoulder & upper arm multiple sites; contusion of multiple sites on right hand and wrist; abrasion of right fingers; head contusion. Employee was prescribed prescription medication for spasms and was placed on restricted duty.

Truck driver was not site specific hazard trained. Road exiting the mine did not have signs warning of steep grade. Truck driver failed to maintain control of vehicle.

Employee was walking out of storage building and his heel caught on the ledge causing him to turn his right ankle.
Employee was hooking chain to bucket to unload. His foot slipped on rung of ladder causing him to fall against the bucket.

Employee was welding on a Haul Pak Truck and when he climbed down the ladder and reached the ground, his knee started pinching and swelling.
Employee was pulling wood from coal and he sprained his lower back.
