MOVING MINER CABLE HURT BACK.
WAS CLEANING UP COAL IN #4 ENTRY. AFTER BACKING UP ON RIGHT SIDE, WAS MOVING TO LEFT SIDE WHEN MINER WENT OVER A LUMP AND RAISED UP BACK OF MINER. HIT HEAD ON TOP.
PULLING MINER CABLE ACROSS A BOOM HOLE DID NOT BEND OVER OF END OF BOOM HOLE TO LOW TOP, HIT HEAD BROW.
EMPLOYEE WAS ADJUSTING BELT WHEN A PIECE OF STRUCTURE THAT WAS NOT TIGHT WAS CAUGHT BY A SPLICE AND THE EMPLOYEE'S RIGHT ARM AGAINST THE NEXT TOP ROLLER.
