Electrician was cutting a 5/8" steel rope cable by hitting it with a hammer. Injured EE was walking by the scoop and a metal fragment struck him in the left eye. The fragment pierced the upper eye lid and required removel and a liquid stitch to close the wound.

A pall was discovered in intake airway 30'l, 20' w, 5-6' thick. Laminated shale with slight water present. Good air flow across fall and a 6' walkway was timbered across fall edges of fall were sloped with scoop to make travelable. Additional supports were installed in roadway and crosscuts between intake & roadway and between roadway and belt.

A crosscut was turned right out of #3 and when crosscut holed into #4 entry the fall occurred in #4 entry in the intersection being developed. Fall approximately 5.5' thick, 20' wide & cannot determine length.
