A 3' x 3' x 8' deep void occurred when floor fell into the lower seam at no. 98 crosscut in no. 3 entry. The air is ingassing with 0.0% Methane and 20.7% Oxygen readings.

EE was operating a gas weed-eater at the #2 Belt. He reached into high grass to remove a rock and he felt something touch his hand. He did not realize he had been possibly bitten by a snake. He continued on weed eating, felt hand tingle about 5 minutes later. EE went to hospital & was kept for observation. EE is scheduled off until 6/3/06, this case may change to a lost time.
ALLEGED to have hit a hole with a coal hauler and hit head on canopy injurying head and neck. He continued working that shift with 5 hours overetime and the 3 additional scheduled days that week. He did not report the injury until 11/01/04 via phone to the buisness manager. His first scheduled workday missed was 11/03/04. This injury is being contested to Workers Comp.

Employee was on the surface of an underground mine when he slipped and fell in mud and ice while taking mantrip off charger, injuring his right shoulder. His first doctor visit was on 2/20/04 and his first day missed work was on 9/14/04. He said he had hurt his shoulder yeasrs earlier in Kentucky.

A VOID WAS DISCOVERED IN THE INTAKE ROADWAY ON THE BELT TUNNEL PROJECT. THE VOID WAS CAUSED BY AROOF FALL ON AN UNDERLYING MINE(WMEFREDE #8) WHICH RESULTED IN THE INTERBURDEN CAVING IN & THUS CREATING THE VOID. THE VOID MEASURED 18' X 23' &WAS LOCATED 83' INBY SPAD # 1139.

A VOID WITH CRACKS HAS FORMED IN THE MINE FLOOR IN THE TUNNEL SECTION OF THIS MINE. THIS ANAMOLYIS LOCATED IN #3 ENTRY NEAR SPAD #1065. IT MEASURES 16'L X 18'W WITH AN UNDETERMINED DEPTH.
A VOID WITH CRACKS HAS FORMED IN THE MINE FLOOR IN THE TUNNEL PORTION OF THIS MINE. THIS ANOMALYIS LOCATED IN THE CROSSCUT FROM 2 TO 3 NEAR SPADNO.1134. IT IS 15' LONG BY 18' WIDE OF AN UNDETERMINED DEPTH.
ROOF FALL 50' LONG X 18' WIDE X 9' HIGH OCCURED IN NO.3 ENTRY NEAR SPAD 1006. THIS SECTION HAS BEEN IDER SINCE 3-31-99. THIS AREA WAS DEVELOPED IN JANUARY 1999.
THIS MINE ENCOUNTERED A VOID WHEN MINING XCUT 2 TO 1 THIS VOID WAS ENCOUNTERED APPROX 65' FROM SPAD #1052. WHICH WAS LOCATED IN #2 ENTRY. THE VOID APPROX 20'W X 50' LO
THIS MINE ENCOUNTERED A VOID IN NO 1 ENTRY WHILEMINING. THIS VOID WAS CAUSED BY A ROOF FALL IN AN UNDERLYING MINE (NO.8MINE) WHICH RESULTED IN THE INTERBURDEN CAVING IN & THUS CREATING VOID
EE WAS RIDING IN A MANTRIP WITH HIS RIGHT KNEE KHANGING OUT OF THE MANTRIP. OPERATOR TURNED THE CORNER TO LET ANOTHER MANTRIP PASS. HIS KNEE CONTACTED THE COAL RIB.
