HE WAS DOWN AT THE EAST PLANT. THE PLANT WAS NOT IN OPERATION. HE WAS UP ON THE ROCK CONVEYOR, CHANGING A ROLLER, THE CONVEYOR WAS OFF AND WAS LOCKED OUT. HE SLIPPED AND FELL SIDEWAYS OFF THE CONVEYOR. HE FELL FOUR FEET AND LANDED ON THE GROUND WHICH HAD 1 1/2" WASHED ROCK ON THE SURFACE. HE CUT HIS RIGHT ELBOW AND BRUISED HIS RIGHT HIP AND LEFT SHIN.

AT THE TIME OF HIS INJURY, HE WAS A LOADER OEPRATOR. HE WAS DOWN AT THE PLANT WHICH WAS NOT IN OPEARTION. HE WAS GOING TO MVOE A BARRELL INTO THE BUCKET OF A TEREX LOADER. HE THOUGHT THE BA RREL WS EMPTY, BUT IT HAD SOME SAND IN IT. WHILE TRYING TO MOVE THE BARREL, HE CAUSED A SLIPPED DISC IN HIS BACK. DUE TO A CHANGE IN PRODUCTION PLANS, HE IS NOW BACK IN THE TRUCKING COMPANY

HE WAS USING A PIPE TO RAKE THE SAND OUT FROM BETWEEN THE ROLLERS ON A 30"X150' STACKER CONVEYOR. THE CONVEYOR WAS NOT IN OPERATION. THE PIPE HIT A ROLLER AND TH ESKIN WAS TORN BY A SHARP EN D ON THE PIPE. SKIN WAS TORN OFF HIS PINCKY AND RING FINGERS ON THIS LEFT HAND.

HE WAS WASHING OUT A NON-OPERATING GRAVEL WASHER. HIS LEFT FOOT WAS ON THE OUTSIDE LEDGE OF THE NON-OPERATING WASHER. HE WAS PREPARING TO STEP DOWN INTO THE WASHER WITH HIS RIGHT FOOT. HE WAS HOLDING THE RINSE HOSE IN HIS LEFT HAND AND REACHING FOR A CROSS-BRACE STRUCTURE WITH HIS RIGHT HAND. HIS LEFT FOOT SLIPPED OUT FROM UNDER HIM AND HIS RIGHT LEG HIT THE PADDLE INISDE THE WASHE

THE WIND WAS BLOWING 35 MPH. HE WAS ON THE LAKE PUTTING IN PIPE ON THE PIPELINE. A SMALL PIECE OF SOMETHING BLEW OFF THE WORKBOAT INTO HIS RIGHT EYE. THE DOCTOR SAID IT LOOKED LIKE A SMALL PIE CE OF STEEL, BUT COULDN'T BE SURE. IT LEFT RUST RING ON CORNEA.

WHILE CROSSING OVER THE BACK OF A CAT LOADER HE STEPPED ON THE TIRED AND SLIPPED OFF. HE LANDED ON HIS LEFT ARM CAUSING A NON DISPLACED FRACTURE OF THE LEFT RADIAL HEAD ELBOW.
