HE WAS BENDING OVER TO PERFORM SOME MAINTENANCE ON THE PLANT STRUCTURE AND TWISTED. THIS RESULTED IN A MUSCLE SPASM, WHICH PROGRESSIVELY BECAME MORE PAINFUL AS TIME PROGRESSED. (MUSCLE SPASMS WERE FIRST DIAGNOSED, BUT FURTHER TESTING IS BEING DONE TO DETERMINE IF THERE WAS ANY DAMAGE DONE TO THE PREVIOUS BACK SURGERY HE HAD DONE IN THE FALL OF 1991.
EE WAS STEPPING OFF THE BACK TAILGATE OF A PICKUP TRUCK. HE TWISTED HIS ANKLE AND FELL DOWN. THIS WAS HIS RIGHT ANKLE. HIS ANKLE SWELLED UP AND HE WENT TO HIS DR. TO DETERMINE WHAT WAS WRONG W ITH IT.
EMPLOYEE WAS DELIVERING A LOAD OF MATERIAL TO A CUSTOMER. HE HAD UNLOADED & HAD GOTTEN OUT OF THE TRUCK & WAS WALKING ALONG THE SIDE OF IT, WHEN HE SLIPPED ON THE ICEY SURFACE. HE WENT DOWN ON THE GROUND WITH HIS LEGS TWISTING UNDER HIM. HE THEN CRAWLED TO THE TRUCK & ALERTED THE CUSTOMER. THE SQUAD WAS CALLED & TRANSPORTED HIM TO THE HOSPITAL.
