A Sein Brute 612 mantrip carrying two miners was traveling at 38-39 xc, 592 S.Longwall Panel 3 Road (1 Room). A mechanical failure within the steering mechanism resulted in the over-turning of the mantrip. A miner was pinned under the mantrip resulting in fatal injuries. The other was ejected, sustaining injuries. The company is investigating this incident with MSHA.

POWER (480 V) TO THE SHUTTLE CAR LOCATED AT THE 7 SHAFT PUMP PROJECT AREA WAS INTERRUPTED WHEN THE GROUND FAULT UNIT TRIPPED. THE CIRCUIT WAS DE-ENERGIZED AND THE TRAILING CABLE, WHICH WAS PAR TIALLY COVERED WITH WATER, WAS HUNG ALONG THE RIB IN ORDER TO DUMP MATERIAL IN RUTS CREATED BY MOBILE EQUIPMENT. POWER WAS RESTORED TO THE SHUTTLE CAR. THE VICTIM CONTACTED A DAMAGED SPOT ON
