Employee removed a guard from a tail pulley area to replace a grease canister while the belt was in operation. The fins on the tail pulley caught the employee's glove, pulling his left hand into the fins, resulting in lacerations and abrasions; which required stitches and a skin graft.

A mechanic was holding a drill steel with the deck bushing raised up on the drill steel and suspended from the jib line. The jib line broke causing the deck bushing to slide down the drill steel striking the employee's hand. The employee's left thumb was pinched causing a laceration needing ten stitches.
Technician lifted cutting edge with bar to put crane sling under it. Cutting edge slipped crushed three fingers on technician's left hand.

While installing top pin on a 785 haul truck the bar holding the strut slipped out causing the strut to fall. The strut knocked the employee off the ladder and to the ground. The fall was 3 feet.
Employee was riding on bus back to office at the end of his shift when bus hit a rough spot in road. The bumps caused the employee to jar his lower back. Employee felt a sharp pain in lower back but felt no medical attention was needed. On June 26th, Employee decided to visit doctor where he was examined and released to full duty.
Operator was driving haul truck down ramp when he hit a soft spot on the ramp. The soft spot caused the EE;'s seat to bottom out. This caused the EE pain in his lower back.
Employee was driving haul truck up an in pit ramp. The front of the truck lifted and returned to the ground violently injuring the employee's lower back. He was taken to the local hospital ER, examined, treated, and released to restricted duty. He is currently working with restrictions for an undetermined amount of time and is under Dr's care.

Empoyee parked haul truck inproperly causing ladder to be higher from ground. EE got off haul truck he felt a pull in his shoulder. He did not feel anything was wrong and did not want to see a dr. He went to dr. 12/12/06 for unrated reason & told dr about shoulder. X-rayed found nothing wrong. still bother him return to dr. if to going for surgery. 4/18/07 he went for medical
Contractor employee was walking along the perimeter fence line of mine property. While walking through the tall grass, employee stepped into animal burrow hole. Employee strained right hip muscle.
The employee reported to fellow workers that he was having problems & suspected he was having a heart attack. The project emergency response was activated with EMT's responding with rescue equipment and vehicle. The employee was transported to local emergency hospital, then on to advanced cardiac care in SLC.

WINCHING A STABILIZER INTO THE MAST OF A DRILL RIG. STABILIZER BECAME HUNG-UP. WHEN IT CAME LOOSE IT HIT THE MAST OF DRILL RIG. HELPER WAS HOLDING ONTO THE MAST WHERE STABILIZER HIT. IT SM ASHED & TORE 3/4 OF THE FIRST JOINT OF LEFT THUMB.
EMPLOYEE SWEEPING THE SHOP FLOOR. HE REACHED UNDER THE STEAM CLEANER TO REMOVE DIRT AND DEBRIS AND HIS LEFT HAND SCRAPPED THE TOP OF A PIECE OF WOOD ATTACHED TO THE CLEANER. THE RESULT IS A SP LINTER PENETRATED HIS FIFTH FINGER BETWEEN THE FIRST KNUCKLE TO SECOND KNUCKLE AT THE HAND. THE DOCTOR REMOVED THE SPLINTER BUT THE WOUND DID REQUIRE 1 SUTURE.
