988 Loader caught fire, was unable to be extinguished by site personnel, 911 was called. Two fire crews arrived on site, one fireman approached the loader by the rear tire. The plant manager was yelling at them to get away from the tire. It exploded killing the fireman by the tire.

10242 CONTRACTING HAUL TRUCK DRIVER EXITED HIS OWN TRUCK NEAR THE OFFICE/TRUCK SCALE. UPON EXITING THE TRUCK, THE DRIVER DID NOT SET THE BRAKES OR THE BRAKES FAILED. THE TRUCK STARTED TO ROLL 15403HE DRIVER ATTEMPTED TO ENTER THE MOVING TRUCK. HE WAS UNSUCCESSFUL IN STOPPING HIS TRUCK AND THE DRIVER WAS RUN OVER BY HIS VEHICLE.
