GUILLOTINE CROSSHEAD FAILED TO OPEN ON ONE SIDE. BUCKET FELL TO SHAFT BOTTOM FATALLY INJURING EMP BEFORE SLACK ROPE IDENTIFIED.EMP WAS ON BOTTOM DECK & PASSED THROUGH BUCKET WELL AT TIME BUCKET DROPPED.
