EE received two fractures in left arm at the shop after the 100 ton haul truck plate popped out and struck him while he was using an air arc to cut floor plate loose.

EE possibly strained stomach after trying to sit up from a flat position on his back while in the pit underneath a PC 400 Rockbreaker to repair a loose wire. EE did not go to the doctor until November 21, 2007.

The employee went to the company clinic on 11/19/07 and was placed on restricted duty. The employee states he used a sledge hammer to remove a wedge that was stuck on the crusher. As he attempted to hit the wedge, he missed the target and hit his R-thumb.

Employee inadvertently contacted his left wrist with a knife that he was holding in his right hand, causing a laceration to his left wrist. The wound required six sutures to close.

Employee requested to see company dr 11-09-2-07, who dispensed medical prescription. He was handing a piece of pipe up to another ee when debris fell and landed into the ee eyes.

Employee was cleaning rail car. Grabbed door lock with hand to unlock when door dropped down catching his finger.

Injured employee was stepping backwards while directing the loader to pick up a pallet, and felt his right knee pop. Employee was not carrying anything, did not trip on anything or was not on uneven ground. Received medical care on 12/06/07 and put on restriction starting 12/06/07.

Injured employee was tightening Pitman wedge bolts while using a knock out wrench to tighten bolts. When he struck wrench with hammer he pinched his left hand between the hammer and wrench causing a laceration to the left hand.

Changing drill steel on drill, he hurt his wrist.

The employee requested to go to the company clinic on 10/19/2007 and was placed on restricted duty. While exiting the VMC excavator, the employee stepped on the last step of ladder with R-toes only, causing an alleged sprain to R-great toe.

A WMC Inc. employee was removeing rollers from a conveyor below the C2A conveyor platform while a Vulcan Employee was removing an inspection cover from a motor on the platform above. The Vulcan employee lost control of the cover striking the WMC employee on his hardhat from a drop of approximately 10-15' striking several points of the conveyor structure on the way down.

EE's left hand finger was crushed between pry bar and a metal skirt, while using a pry bar to clear a rock out of the plant feeder at conveyor #2, receiving ten stitches.

There were stones present on the ground around Bin #8 due to spillage. While walking toward the stairs, Employee was inattentive to the condition, stepped on a stone causing a fall to ground, injuring left ankle. Employee finished the shift. The next day Employee went to a clinic and an x-ray was taken, confirming a fracture.

The employee went to an emergency room on 10/05/2007, received stitches, and placed on restricted duty. The employee was cutting zip ties with a box knife cutter and lacerated his left forearm.

Employee was washing rock from elevated catwalk with a high pressure water hose. The trailing hose slipped off the catwalk, pulling the hose from employees hands. The hose whipped around and the nozzle stuck him in the lower back on the right side. Injury was reported the day of occurrance, did not seek medical care until 10/5/07

The injured was handing a rock to a co-worker while inside the crusher clearing it out. It slipped out of the co-worker's hand and fell about two feet - hitting the injured's left shoulder.

Employee was descending the ladder from his CAT 992G Loader (at the end of his shift). As he was stepping down from the last rung to the ground (using his right leg) he heard a pop in his left knee that was still on the last rung of the ladder (the swinging rung).

The facility was hosting an open house for the local neighborhood, when the injured was attempting to go to the porto potty, when she slipped and fell.

EE right thumb was crushed while trying to position a stack of plates on the vertical tilt band saw. When closing it the thumb was crushed between the hydraulic vise and the plate.

The employee felt pain on 10/01/2007 but did not report it until 10/12/2007. He was then taken to the company clinic and placed on restricted duty on 10/12/2007.The employee states that driving the pit truck caused pain and numbness in his R-arm, R-shoulder, and R-thumb allegedly.

EE was using a prybar to remove a concave inside a gyratory crusher. The prybar slipped and caused the EE to fall forward against the crusher head and hurt his right shoulder.

The inside of an employee's left knee was hit by a rock that bounced off #3 crusher while he was cleaning with a water hose.

While removing a part from the haul truck, pressure from suspension was released, allowing truck frame to fall.

Employee cut middle and ring fingers of left hand on squirrel cage (blower motor) on 988F. Hand slipped while performing maintenance.

He was changing a cable on an automatic recoil device on a crane tagline. When the dogclutch of the recoil device was released, we believe the cable caught employee's right leg and threw him into the air. He received contusions and abrasions to his right ankle, knee and shoulder.

Employee was walking down a wooden ramp leading from a meeting room. He twisted his foot, which resulted in a broken bone in his left ankle.

Hose blew off the steamer @ heater coil. Employee was burned/scalded.

Employee was removing wheel cover on haul truck. While pulling a wrench to loosen bolt in cover, the wrench slipped and he struck his wrist on lug nut.

EE has a reportable hearing loss. EE experienced a reportable injury to his right ear on 10/17/05.

EE sprained his right knee after his foot slipped off the 600 Komatsu Loader step ladder, falling 6-7 feet and landing on his right foot.

EE was changing the drill hammer. He broke the hammer loose and began replacing the chuck, when the hammer unit fell, catching his left index finger between the hammer unit and the drill deck.

The EE's left middle finger was fracture and lacerated, with three stitches, after removing bolts from an planetary assembly spindle that fell on it. EE was not wearing required gloves.

The employee went to the company clinic on 8/24/2007 and was placed on restricted duty. The employee was bitten on back of right knee by a spider while refilling the water truck or while in the cab of the water truck.

The employee requested to go to the company clinic on 8/23/2007 and was placed on restricted duty. While moving a shaker screen, the employee slipped on a loose pile of rock resulting in a R-inguinal pain (ALLEGED).

Employee was manually moving a piece of 100-250 lb. rip rap when another empoyee rolled a second piece of 100-250 lb. rip rap on his finger. His finger was caught between the two pieces of rip rap.

Employee cut his right arm when he jerked a pneumatic grease pump (app. 30 lbs.) out of an empty grease barrel. The pump apparently hung on something in the barrel. When the pump came loose, it hit the employee in the right arm, causing a laceration that required 4 stitches.

Employee had a periodic screening chest x-ray read by two "B" readers using ILO procedures. The x-ray was characterized as being consistent with pneumoconiosis with reading of 0/1 and 1/0. Damage or impairment cannot be determined by reading chest films only. This employee will be sent for a comprehensive respiratory examination

Working on crusher, he was struck by falling slag.

Employee was using a wrench attempting to break a bolt loose with body in full tension and body in prone position approx. 6-8 inches off the ground. When the bolt broke loose, he struck the ground, buttocks first, but sustained no apparent injury. He awoke the next morning with severe muscle spasm in lower back.

Employee exited from his pickup tuck and made an attempt to close the door when he tripped and fell over a depression in the dirt. His hand was caught between the door resulting in two stiches.

Employee was using a pry bar to help adjust a tail pulley on a stacker belt. His finger became wedged between the pry bar and the stacker belt framework, which resulted in a finger being broken.

South Alabama Drill employee was changing a drill bit and caught the tip of his left ring finger between bit and chuck.

During the removal/repair work of the primary crusher, the concave was being hammered on using the rock breaker. The employee remained on the crusher floor to view the operation from about 12 feet away. He was struck by a piece of sharp packing material in his right inner thigh. He received three stiches and returned to work.

Employee was installing an electric motor; motor moved quickly and pinched his hand between motor and I-beam structure.

Employee was replacing the master pin in the track on a Komatsu track hoe. A piece of the pin chipped off striking the forearm of the employee.

After cleaning out rail car; employee was using a metal bar to close the rail car door. He said something let loose in his lower back. The door was defective and hard to close.

Employee was removing a piece of tramp metal from the Nordberg 7' standard crusher. He was inside the crusher using a burning bar to remove metal. When metal gave way, a piece flew out and struck employee on the right hand and under his chin.

The injured employee was trying to remove a piece of metal from the #3 belt when the magnet pulled the metal piece back up and caught the tip of the right index finger causing a amputation to the tip of the right index finger

Supervisor/employee was loading screen tubes onto a boom truck with 2 other employees. The supervisor/employee caught his finger between one of the screen tubes and the boom truck.

A splinter entered the EE's left forearm when he brushed against a rail tie while assisting another EE load a front-end loader onto a lowboy trailer.
