Cruz Mejia, a 29-year old laborer with nine weeks of experience, died on October 25, 2018, when his escort vehicle flipped while he was ascending a mine haul road. The victim's vehicle veered off the haul road and climbed the adjacent haul road bank, causing the truck to overturn.

Findings from MSHA's final investigation report, which supersedes the preliminary narrative above.
