An unplanned roof fall occurred at 11:25 a.m. in the 1st East Panel 1 at spad 10+80, the roof fall was above anchorage, and continued into adjacent intersections. This is a worked out area, no escapeways or travelways effected. Floor to roof support were installed and area dangered off. This fall will not be loaded out.

A fall of roof occurred in the return entry of the 1st North Submain # 7 xc, entry # 3. The fall was approximately 50 long x 18.5 feet wide and the thickness was unknown.

Production hoist head rope #4 was found to be out of spec during regularly scheduled NDT testing. The hoist was taken out of service and work is underway to replace the production head ropes.

An unplanned roof fall occurred inby the section tailpiece at the 36 Bk intersection in the #4 entry on the D-Mains (MMU 001-0) section. There were no injuries or disruption to the ventilation system, but the remote controlled continuous miner is under the fall. This occurrence was immediately reported.

While completing a post blast examination a ground failure was discovered in the area where the blasting occurred at the 5600 Joyce South. Deteriorating ground conditions over time combined with recent blasting of material contributed to the failure.

Employee was traveling down in a passenger elevator and it stopped between the third and second floor. Elevator service company called/responded. MSHA also called because it would take longer than 30 minutes to get the employee out. Employee out in approximatively two hours. 19 Communication maintained. No injuries.

Miner was in the cement silo elevator when it unexpectedly stopped. EE was unable to get out of the elevator, waited for the service maintenance company to arrive. EE was in the elevator longer than 30 minutes prompting an immediate notification call to MSHA. The call was made in the allotted time and the miner was shortly after able to exit the elevator.

During routine maintenance on longwall equipment, a methane release was detected by monitoring equipment. Area was immediately evacuated and ventilation controls were adjusted to dilute the liberated gas. Gas continued to be emitted so as a precaution the mine was evacuated. MSHA was notified as courtesy through the 1-800 #.

An unplanned roof fall was discovered on #3 belt line at 53 block. The fall was approximately 35' long and 9 1/2' high. There was no one injured in the fall.

Found an unintentional roof fall at break #33 on the Hall Mains-Track Entry. Fall dimensions were approximately 20 long by 20' wide by 6 feet thick.

An unplanned roof fall occurred above the anchor point in the #24 crosscut on #11 Mainline Track between the #1 and #2 entries. The fall was approximately 60 feet in length by 16 feet wide and 8-10 in height. There were no injuries involved with the incident. The fall does not affect ventilation. The crosscut is not used for passage.

Module in hoisting system failed causing the hoist to stop between levels. Miner was trapped for more than 30 minutes. No injuries.

A contracted mechanic was conducting hot work activities on a drill rig, in close proximity to three freshly drilled bore holes. As EE was using a cutting torch, EE alleges to have heard a noise come from one of the holes. There were no open flames or any material ejected from the hole.

The elevator was inoperable for an extended period due to an over travel fault. It was found that the elevator did not recognize one of the landing sensors. The sensors were cleaned or replaced and the elevator was recycled. No one was on the elevator at the time.

Roof fall occurred on Main North travel road, x-cut #5, entry #3. An action plan was submitted and approved by MSHA.

A fall of the roof occurred in the 3rd NE Mains, 4th North Intake, crosscut 46, from #3 entry toward #2 entry. The fall measured approximately 50ft. in length, by 20ft. in width, by 7-15ft. in height. This area has had additional standing support installed and the primary escape way has been re-routed.

An unplanned roof fall was discovered in the #4 belt line at 20 block. The fall was approximately 36' long x 8-10' high. No one was injured during the fall.

The continuous miner was conducting retreat mining on the 2nd left panel off of Beast Sub-mains when the continuous miner got stuck and then some rock fell and caught the continuous miner preventing it from tramming out of the cut.

IGNITION - Occurred as the headgate drive was raised about 7"" above the foot wall when a methane/frictional ignition occurred under the panline at headgate drive from the longwall face chain and was extinguished with water.

An unplanned roof fall occured at 22.5 crosscut of the North Mains Belt line. The fall measured 27' long, 16' wide, and 7' above the mine roof. No one was injured.

A non-injury accident occurred on the mines raw coal stock pile. An employee crossed an unknown void in a Cat D9 dozer causing the dozer to fall into the unknown void entrapping the miner.

An unplanned roof fall was discovered in the #4 belt line at 17 block. The fall was approximately 48' long x 8' high above the original roof line x the width of the heading. It did not impede ventilation and no one was injured.

An Ignition of methane has occurred in the No. 5 face on the 5 left panel. A methane bleeder had been cut into by the continuous mining machine. A spark created by a miner bit against the sandstone roof caused the ignition. The flash of methane was quickly extinguished by the water sprays which were on at the time of the incident.

An unplanned roof fall occurred above the anchor point at 2-32 3/4 crosscut to 2-34 crosscut. The fall was approximately 160 feet long by 16 feet wide and 8-10 feet high. There were no injuries involved with the incident. The fall does not affect ventilation but it does impede passage. The fall will be supported to gain access to travel the air course.

Employee was operating 988 loader, when one of the rock trucks was backing in to get loaded, got too close to the loader boom and caught it with the bed and turned the loader over. The loader operator radioed the rock truck driver, but no communication was received by the rock truck driver. Upon investigation, the external speaker in the rock truck had quit working.

An unintentional roof fall was discovered by a Mine Examiner on the Lyons Mains Section. The roof fall was in the #6 Entry between spad #s L416 and L462 and measured 82 ft. L x 18' W x 12 ft. H. No people or equipment were damaged or ventilation disrupted.

An unintentional roof fall occurred in the #4 Belt Entry at Crosscut #11 on Hall Mains. Roof Fall was approximately 20' wide by 50' long and 12' thick.

While loading the face with explosives the air compressor on the powder trailer unexpectantly caught fire. The nearby employees quickly shut off the tractor powering the compressor and made the decision, with their supervisor, to evacuate the mine.

A roof fall was discovered on this date. The fall measured 10' by 20' and was 10' in thickness. The fall was located on 3-North-3, crosscut #83 in the #6 entry. No persons or ventilation was affected.

While x-raying the hoist rope, two broken wires were found within a single strand. The hoist was taken out of service and the rope was replaced.

On 10/12/18 an employee was cleaning to drill a slab round on the rib with a mucker. When ee rolled the mucker bucket back a small pop was heard. No injuries or damage was reported. The employee did not report the incident to management until 10/15/18.

A personnel elevator in the preheat tower stopped unexpectedly and 3 miners in the elevator were unable to get out for 48 minutes. The supervisor on duty called the immediate reporting hotline within 13 minutes of notification. A report was sent to the district office in Lansing.

A roof fall occurred in the #1 entry, 8 East section, at crosscut 31+25 extending approximately 70 feet outby. Approximate dimensions were 70 ft long by 22 feet wide by 6 feet high. The area was bolted with 6 foot long resin encapsulated rebar bolts.

A roof fall occurred in the 9 seam on the 8th 54 beltline between xcuts 7 & 8. The roof fall measured approximately 12'L x 6'T x 18'W. The fall will be cleaned up utilizing the MSHA approved roof fall clean up plan. The roof was supported with 6' fully grouted and 8' mega roof bolts. Date of development 3/2010.

69 caps and boosters were shown on the blast report for the blast on 9/27/2018. After investigation, the blasting contractor determined that 68 detonated during the 9/27/2018 shot.

The mine electrician was doing a daily examination on the Miracle Run Elevator, when they tripped the safety device, causing the cage to lock-up. Schindler Elevator Services had to be called to release the locks. No one was injured, and the employee was off the elevator at 2:45 pm.

An employee was loading haul trucks using the CAT 365B track hoe when the employee noticed the machine had caught fire. The employee exited the machine and called for assistance. The fire department extinguished the fire. The cause of the fire remains undetermined, and there were no injuries suffered.

An unintentional roof fall occurred in 3 East Mains 2 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are humidity in the area and adverse roof conditions.

An unplanned roof fall was discovered in the 1st south sub-main intake out-by area at spad # 0+00. The area is not traveled except for examinations, no ventilation disruption experienced due to roof fall. Area previously supported, at time of mining, with 6' fully grouted bolts and supplemental supports consisting of 9' cable bolts.

An unplanned roof fall was discovered in the 16-M to 17-M walkway at 15 blk. The fall was approximately 20' long, 16' wide and 4' high. The fall was most likely caused by gradual deterioration of the entry.

Employee was completing security roving patrols in vehicle and blacked out causing the vehicle to wreck into out of commission scoop on haul road. Vehicle will be totalled due to excessive damage on passenger side including doors, fenders and bed of truck ripped and torn off.

A roof fall occurred when the mine was idle. It was discovered around 10:45 pm on 10-7-18. It occurred in room #3 of the 2nd West panel at the 240' station. The fall was 35' long, 18' wide and 3-4' thick. It didn't affect ventilation, passage of men, block an escape way, or injure anyone.

No. 3 Hoist computer screen locked up, per instructions on screen - breaker was reset. Breaker would not reset - electrician turned main breaker off; everything reset.

On 10/5/2018 at 3:55 pm the mine operator was made aware of three contract miners were stuck in the 5600 East 13800 Alimak raise due to a mechanical failure to the Alimak air motor & did not believe repairs could be made within the 30 minutes. MSHA hotline was called & notified by the operator at 4:13 pm. Repairs were made & miners were out of the raise at 7:30 pm.

An Unintentional roof fall occurred on the Grassy Creek Mains, 2 East Mains Development section Brk 21 in the #6 entry at SS #10470. The fall is Approximately 60 ft to the North and 60 Ft to the Northeast of SS# 10470 the fall height is unknown.

An unplanned roof fall occurred on the #7 belt track entry in the #37 intersection 40 back toward the #36 intersection in the #2 entry. The fall was danger and timber off. The track and life line was run to the #1 entry. The fall will be cleaned up to the rehabilitation cleanup plan.

A Roof Fall in the GOB area of the 2nd East Retreat (Partial-Pillar) section occurred that damaged, or knocked-out, as many as 37 Ventilation Controls, (Stoppings, Regulators & Overcasts). This caused the Air to reverse direction and/or short-circuit. It also caused Rock Dust to be suspended in the mine's air courses for several minutes.

An unplanned roof fall was discovered in the 4 West #3 Entry at 24 1/2 to 25 block. The fall was approximately 50' long, 16' wide and 10' high. The fall was most likely caused by gradual deterioration of the entry.

An unplanned roof fall occurred in the 11 Mains North section. The fall was in the #4 entry between 13xc and 14xc. The fall was approximately 25 foot long from the middle of the entry to the left rib. It is approximately 8' high, which is 2' above the anchorage point.

Employee was in an articulating haul truck, was traveling down the haul road at the Quarry. The victim was moving the haul truck down a steep grade, around a turn, when the haul truck traveled through a berm and off a short drop-off. The victim was found hours later by the owner of the company, the victim was not wearing a seat belt.

A 33 year old miner was attempting to move an auger steel with an onboard crane when a section of the auger struck ee chest. EE was transported by ambulance to a hospital where ee died.

EE was in the process of opening a 6' thick slab. Water bags were installed at the top of the slab. EE was not wearing a safety harness and was about 20' high. Slab broke and fell down & hit EE and crushed him.

Struck by flying object

An employee was attempting to clean out a previously blasted vertical drill hole with an air hose. A sudden release of pressure from an unknown source inside of the hole forced material to be ejected from the hole and struck the employee. The employee passed away from the incident.

Caught in, under or between a moving and a stationary object

While installing a cable crossing, employee's right foot was caught under the crossing while a Rubber Tire Dozer was pushing it into place. Within approximately 5 minutes the employee's foot was released. Due to the gravity of the injury, the employee underwent a partial amputation of the right foot after being evaluated at Wyoming Medical Center in Casper, WY.

Employee lost control of 242D skid steer. Employee's negligence resulted in multiple collisions resulting in their LH index finger tip being amputated. Employee reported injury to another the lead person & was asked what happened they stated, ""I put my hand where it doesn't belong.""

Employee was leveling a bowl liner to get ready to pour and bowl liner came off forks and pinched between mantle and cribbing

Struck by falling object

Employee was in intersection waiting for the belts to come back on when a section of roof rock fell striking ee in the left foot causing a crushing injury and amputation of three toes. the rock measured 37"" wide 67"" long and 11"" thick.

Fall onto or against objects

The employee called control room to shut off 2 belt through 4 belt to inspect the sampler return chute. The employee climbed up to stand on the hand rail and leaned over the walkway using hands to steady themselves on the Chute wall. The employee Inspected the chute, went to push off and slipped causing their hand to slip into the chute causing the injury.

While making shop repairs to a spare mixer assembly, technician turned the internal rotor. The rotor became momentarily seized and released before the technician could remove their hand. Finger caught between the rotor assembly and the lid, causing the loss of a portion of the distal phalange, 1/3 of the fingernail, on the left ring finger.

The dust collector for the feldspar process collects fines in a bulk bag. The operator was making adjustments to the bag when operator slipped and fell. Operator put left hand out to catch self and caught a finger in the chain drive for the airlock.

Employee was riding on the side of the lead car in a string of cars when EE slipped while dismounting and fell under the car. The car ran over EE's legs severing both legs.

An employee was working with a Southwire M6K Maxis to pull wire. The individual got the left little finger caught in the rope; the employee couldn't get finger out or shut the tool off in time, amputating the top of the employees' fifth digit.

Miner bypassed the guard to access V belt on the head pulley Motor belt. After checking the tension of the V belt with fingers, the operator in the tower started the belt without sounding the alarm. This resulted in the Miner getting fingers entangled in the belt and pulley. The miner lost the tips of three fingers.

Employee lost footing, tried to catch self, and wedding ring got caught on a bolt on the windshield of motor. This resulted in skin tearing. The finger was later amputated, at the hospital, because of a lack of circulation in the finger.

Over-exertion in lifting objects

EE was loading 5'resin in ee scoop bucket when ee felt a pain in left shoulder. EE had already loaded 20 bundles of glue before the incident.

Over-exertion in lifting objects

EE was assisting to lift a roof bolter drill head approx. 24 inches off the floor into a personnel carrier so that it could be transported to the surface when EE experienced extreme pain in EE's lower back. The drill head weighed approx. 140 lbs. The injury occurred at the end of the section supply track way.

Struck by flying object

While removing the front pad housing steel from the double boom face drill the housing steel jumped up and struck the employee on the left eyebrow.

Employee was making sure that a tie-down strap was secure to truck. When ee pulled on the strap, it was loose, ee lost balance and fell. As ee fell the strap caught and pulled arm. This caused rotator cuff on right shoulder to tear.

IE was preparing to weld a steel liner onto the plant feed flop gate. The plate was lowered part of the way into place. As the IE was lowering the plate into place, left ring finger was lacerated and required 7 sutures to close.

EE was operating a walk-behind skid steer and was pulling backwards to be able to use the bucket to scoop up another load of material off the floor when EE's left shoulder made contact with a steel I-beam causing arm to jolt forward into the equipment controls injuring the wrist.

Employee got motor at 1 West Track 76 block and was walking along the track not paying attention to the pavement and twisted EE's ankle.

Employee was handling roof mesh and smashed finger between mesh and bolt.

EE was in #1 screen house sealing up cracks with caulking. EE had just finished the task and was on way back out when EE misjudged a step and fell forward landing on the catwalk edge cutting open left leg.

While chipping a weld, a piece of slag ricocheted off employee's forehead went behind their safety glasses and struck their left eye.

Over-exertion in lifting objects

As an employee was moving a bottle jack, they felt pain in their arm. It was determined that a tendon was torn. On 11/26/18, the employee had surgery to repair the tendon.

Employee was walking through a crosscut when ee stepped in a rut causing to roll ankle.

Fall onto or against objects

Employee was changing clothes prior to the end of their shift in locker room. While removing their pants, the employee's foot got caught in the pant leg. The employee fell over and struck their head on the edge of a locker. Employee received stitches and staples.

Ingestion of radiations, caustics, toxic and noxious substances

EE went to work truck to get a drink & grabbed a 1-gallon windshield wiper fluid bottle from the bed of the work truck that had contained antifreeze (the bottle was labeled in ""antifreeze"" in sharpie) believing it was a Gatorade mix bottle. EE poured some into EE's water bottle and drank half the bottle before realizing something was wrong. Employee was advised to go to hospital.

Employee stated was walking around a forklift charger and stepped on a rock and lost footing causing EE to twist knee. On 11/1/2018 Employee sought medical care and was placed off work under doctors care.

Struck by falling object

When lifting scoop battery lid to vent gases when charging. Employee was using left hand to lift lid and right hand to position lid support. Employee thought support was in its slot to hold lid and left go of lid dropping the battery lid on right arm.

Caught in, under or between a moving and a stationary object

EE was holding onto an area where a ""Nut"" was attached with a wrench while another employee was using another tool a ""Power Wrench"" to loosen the nut. The vibration from the ""Power Wrench"" caused ee to loose grip, the wrench ee was holding onto ""slipped"" and the result was a ""Fractured finger"".

Over-exertion in lifting objects

As the employee was hand loading bundles of glue onto a roof bolter, EE felt a sharp pain in the lower back and shoot down the left leg. EE's doctor believes EE has a pinched nerve and is expected to make a full recovery without requiring surgery. EE began missing work on 10/30/2018.

Flash burns (electric)

Locking out power to wash screen, when panel arc flashed and the injured employee suffered 2nd degree burns to back of hand and 1st degree burns to face and arm.

Gripping Jaw clamp was being used to position conveyor belt. The clamp slipped off of belt and contacted employee's hands fracturing right pinkie and left thumb.

IE stated picked up a bundle of bolts, began to turn towards the hauler, feet got tangled up on a pallet of supplies and started to fall. During the fall, IE tossed the bolts and tried to catch balance. Left arm/shoulder twisted and rolled in a backward motion. IE was brought out of the mine, taken medical facility & arm placed in a sling, with no use of left arm.

The employee had been in a sitting position for roughly five minutes, when they stood straight up and immediately experienced a sharp pain in their lower left back. The employee could not stand straight afterwards, without continually experiencing pain.

Employee was in the right side return airway coming off of overcast when EE slipped on steps and hurt back and neck.

Employee stated EE was at the feeder attempting to restart the feeder when the left S/C struck EE.

Employee was removing bad belt frame when EE mashed left index and middle finger.

Over-exertion (Not Elsewhere Classified)

Employee was operating a cement mixer at a crib plant. EE stepped off of the operator's platform and twisted right knee. Please note that the employee continued to work unrestricted until 11/2/2018. Officially off of work 11/5/2018.

Struck against stationary object

EE was constructing permanent stopping when EE sprained knee, EE seeked medical attention on a later date becoming a RLT on 11/1/18. Cause of this incident was lack of employee concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.

Struck by flying object

A fully trained miner was rock dusting the belt when the pressure was turned on the duster the hose struck ee in the left knee. The injury was not the result of any compliance issues nor were any protective guards involved.

Contact with hot objects or substances

A fully trained miner was welding when a piece of hot slag went into their left ear. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck by falling object

EE was tightening the cats on a continuous miner (CM). As they were positioned on the mine floor beside the CM, a piece of loose draw rock that was resting on the top of the CM shifted and fell striking them on the right ankle/foot, causing a fracture. They saw the loose material prior to squatting down, but did not think that it would move. EE began missing time on 10-29-18.

Employee stated EE was helping move CM cable out of the way when MRS cable pulled down and struck employee in the back of the head straining EE's neck. Employee sought medical treatment on 10/30/2018 and was given three days away from work.

Employee was cleaning beside crusher. A rock rolled over feeder edge and fell striking employee in shoulder. Do not know extent of injuries at this point. Update: Shoulder was bruised, no broken bones.

Employee was riding on a man bus when the bus came off the track. Employees were trying to jack bus back up on track. Employee was using a jack and the bar slipped out of the jack causing employee to fall and fracture their right leg.

Struck by powered moving object

While tightening all-thread on steel set, employee was struck by material on the conveyor belt on the wrist, causing them to lose their balance and strike their shoulder on the beam.

Employee was placing pressure on a pin on the long arm to get the pin past the notch to position it on the chuck. The pin slipped off the long arm and snapped back striking employee's right hand. Employee was taken for examination and returned to work next shift and worked until a recheck of the hand on 11-18-2019, at which time employee was placed off.

The water car was not uncoupled and was pulled over top of employee #1's foot when the motor moved forward.

The injured employee was working with another employee, loosening nuts on a jaw motor to adjust it. The injured employee was leaning forward, pushing on a wrench when the wrench slipped. This caused the employee to be forced forward over the side of the jaw frame where the employee fell on the dirt. The employee was about 5 feet off of the ground.

Employee was cutting a strap off a bundle of 4 foot bolts with utility knife, sat knife on the scoop with blade still out, pulling on bolts slipped cutting forearm with knife.

EE had noticed a belt roller that was frozen on the belt line. X-rays revealed a small fracture in the scapula. EE also had swelling in the arm that required surgery to remove pressure.

Employee was at sand & gravel plant and went to pull lever, slipped, and fell. EE put out hand to catch self and the left hand hit a few crushed #2 gravel on deck and cut hand.

While in the process of drilling a bolt hole, when a piece of rock fell from the roof ,striking the canopy of the roof bolter and a piece 12""X 10""X 1-0"" struck right hand's ring finger causing an avulsion of the of the skin from the very tip of finger. On 10/10/18 at approximately 1130 am the operator was operating the bolter in 16N22. Installing 8 foot roof bolts from the basket of the roof bolter. While inserting the 7th bolt, the lift cylinder bottom mounting pin boss had a structural failure causing the boom and basket assembly to fall approximately 18 feet, to the ground. The employee sustained multiple injuries.

EE was pulling excess curtain from the mine floor to advance curtain on bolt cycle in the face. A rock measuring approximately six inches wide by seven inches long by three inches thick dislodged from the mine roof and struck ee on left shoulder. EE finished shift but went to the emergency room later in the evening with complaints of shoulder pain.

Employee was mowing the grass and brush on berms. While moving the mower across the top of a berm towards the cab the blades struck a roof bolt that was lying horizontally under the brush. The roof bolt traveled end first through the windshield and struck ee in the tip of left thumb. It came to rest across lap.

Contract employee was stepping back away from a customer sand truck to look at the top hatch and tripped over a concrete footer. This caused EE to stumble back and come down on the right ankle then EE's body twisted and the foot did not, causing an ankle fracture.

I.E. was bolting when a piece of shale fell from the roof. The I.E. saw it coming and twisted to get away from the rock. While twisting to get away they fell into the side of the bolter and felt a pop in their lower back. Comp deemed this injury as pre-existing, reporting due to lack of evidence to support.

The Chain Jack was being used to horizontally suspend the tail pulley assembly. The chain link gave away which immediately caused the tail pulley assembly to release horizontally and pin EE's hand.

While connecting discharge water pipeline together employee pulled the collar from the bottom of the pipe around to secure the lock straining shoulder.

Employee was given restrictions on 11/5/18 making this an MSHA reportable. Employee felt discomfort in the left knee while turning body and lost balance. When falling, employee struck the lower part of the left thumb on a grinding ball. Employee reported both knee and thumb pain.

Employee was working on a belt splice replacement. As EE was pulling on the belt, EE felt a pain in the right shoulder.

While lifting a 5 gallon can of fuel a miner felt a pain in the groin area. EE was examined at a Medical Clinic and told EE may have a small hernia. They set up an appointment to see a specialist and released EE on Modified Work Release. The full Diagnosis has not been received at this time.

Employee was clearing a jam on the thin stone saw. Employee did not lock out machine when opening the door. Employee used the belt and leverage to try to free the caught stone and caught glove behind stone and pulled hand between belt and non-operating blade.

Descended ladder and stepped onto concrete floor, rolled ankle, lost balance, fell onto equipment stand, striking left side rib cage. 40 hours later, they felt sharp pain. Evaluated at local hospital emergency room, diagnosed with a fractured rib. EE was off work for two days, then released to modified duty (lifting restrictions).

Employee removed the protruding part of the bit at which time the hammer was sat down accidently firing the mechanism. NAIS crew was hydroblast cleaning the inside of tank. EE wanted to shotgun a different area of the tank but there was a slab of material approx. 30-40 lbs where EE wanted to stand. EE tried to pick up the material to move it out of the way. EE lost control of grip and the material fell on EE's right hand. EE was able to get hand out from underneath the material and exited the tank.

Struck against stationary object

Changing shank on loader. In order to repair, had to remove wear plates that were bolted in. Using impact to remove nuts from bolt, cut palm on bolt. Received 12 stitches in right hand palm.

Caught in, under or between a moving and a stationary object

Employee was handling pipe and smashed finger causing injury.

Fall down stairs

10/24/18 employee fell when walking up stairs to start the crusher & hurt left wrist. Employer did not know about this incident until 10/26/2018 when employee was told ee was terminated for job performance & then the employee told manager about the incident and that ee had gone to the doctor. Employee was not terminated for this incident.

Struck by falling object

IE was roof bolting when a piece of rock fell loose and struck IE in the mouth causing a front tooth to break off just below the gum line.

Employee was cutting a piece of belt when the knife slipped and cut left leg requiring 4 stitches

Caught in, under or between running or meshing objects

Using porta-power & press pin to push pins out of drill roller frame. Holding pin w/left hand & operating press w/righthand. Left hand slipped off press pin & pinched left pinky finger in between press & press pin.

Struck by falling object

While a maintenance employee was being hoisted in a man basket after doing repairs in a leach tank, ee looked up to ensure the man basket was clear and some dust or debris fell from above landing in right eye. EE was wearing safety glasses. While trying to remove the particle it scratched cornea.

Struck against a moving object

Employee was driving endloader on uneven surface causing jerking motion after hitting a hole giving EE whiplash.

Struck by flying object

IE was installing roof bolts. As they pushed a roof bolt up into the hole the bolt bent causing pressure on the drill wrench. The drill wrench broke striking them in the mouth. Employee received 1 broken tooth and a laceration on their lip requiring 2 sutures.

Over-exertion in pulling or pushing objects

Pulling shot hose out of face - working on shot crew.

An employee was using a box cutter to remove packing material from belt. EE inadvertently made contact with EE's left leg. It resulted in a 3/4"" cut to EE's inner thigh.

Fall from ladders

Employee fell from a ladder and broke wrist while adding washers to bolts on a structural steel building that was under construction.

Employee slipped getting off a loader. EE had contusion of the right knee.

Struck by flying object

5200-206 glue back hole with dwyidag, PVC pipe slipped out of hands of miner; striking upper lip, laceration.
