THE VICTIM WAS STANDING ALONG THE LEFT RIB SIDE OF THE CROSSCUT 4 TO 5 ENTRY APPROX 20 FT OUTBY STATION #34751 OF THE 5 RT 002 SECTION THE VICTIM WAS STRUCK IN BOTH LEGS WHILE THE JOY 10 SC -2 2 SHUTTLECAR WAS ENROUTE FROM WORKING PLACE TO THE SECTION DUMPING POINT THE VICTIM SUSTAINED AMPUTATION OF BOTH LEGS IMMEDIATE FIRST AID WAS ADMINISTERED AND VICTIM WAS EVACUATED TO HOSPITAL
