WHILE MAKING HOIST REPAIRS MAIN POWER CONTACTOR WAS OPENED AND ROLLED OUT ON TO FLOOR TO CHANGE COIL APPARENTLY OPERATING LINKAGE HAD STUCK NOT ALLOWING MECHANISM TO OPEN ALL THE WAY TO THE LATCHED POSITION AND WHEN CONTACTOR WAS PUSHED BACK IN IT OPERATED THE SWITCH LINKAGE IN REVERSE PULLING CONTACTOR IN TO HOT BUSS CAUSING FATALITY
