A ROOF FALL OCCURRED IN THE 2 WEST SECTION IN THE INTERSECTION OF THE #2 ENTRY INBY THE FEEDER BREAKER. THE FALL AREA HAD BEEN MINED TO THE ROOF ROCK & BOLTED WITH 4\ MECHANICAL ROOF BOLTS ON 5\ CENTERS. THE FALL TURNED LOOSE APPROX 4\ TO 5\ ABOVE THE ANCHOR ZONE OF THE ROOF BOLTS. ONE PERSON RECEIVED FATAL INJURIES RESULTING FROM THE FALL.
