EMPLOYEE DISREGUARDED SUPERVISOR\S SPECIFIC INSTRUCTIONS AND FELLOW WORKER\S WARNING, CLIMBED OVER OR UNDER A CHAIN LINK FENCE AND GUARD RAIL ENCLOSING V-BELT DRIVE FOR SLURRY PUMP TO PERFORM WORK IN AN UNAUTHORIZED/RESTRICTED AREA. HE WAS CAUGHT IN BELT DRIVE AND WAS FATALLY INJURED.
