WHILE UNLOADING A PALLET OF CEMENT BAGS FROM THE WEST SKIP WITH A HI-LOW, WOODEN PALLET BROKE CAUSING CEMENT BAGS TO FALL TOWARDS THE EAST SHAFT. VICTIM WHO WAS BELLING SKIP, MOVED BACKWARDS T O AVOID BEING STRUCK WITH FALLING BAGS OF CEMENT, LOST HIS FOOTING AND FELL APPROX. 35 FT INTO THE EAST SHAFTWAY.
