MINER RECEIVED BLOW TO RIGHT SIDE OF FOREHEAD WHEN HYDRAULICALLY POWERED DRILL PROPELLED BOLT WRENCH OUT OF THE DRILL HEAD SOCKET. HE DIED FIVE DAYS LATER.
