FOREMAN I.NAME HAD FAILED TO DUE AS RECOMMENDED BY SUPT. HE FAILED TO CLEAN UP ROADWAY INTO #5 ENTRY AND SET REQUIRED TIMBERS NEEDED IN AREA, AND IT APPEARS HE HAD TAKEN TOO MUCK COAL IN AREA AND CAUSE A DANGER FOR HIMSELF AND HIS MEN, AFTER WHICH HE HIMSELF BECAME A FATALITY ON 4-16-84 BY A ROOF FALL IN #5 ENTRY 1ST RIGHT
