THE VICTIM WAS SITTING IN FIRST CROSSCUT IN THE #1 ENTRY ON THE FIRST LEFT SECTION 60\ INBY SPAD #67. THE ROOF IN INTERSECTION FELL ABOVE ANCHORAGE ZONE OF THE ROOF BOLTS RENDERING FATAL INJUR IES TO HIM.
