OPERATOR WAS LOADING 85 TON HAUL TRUCK HAD PUT IN 6 LOADS WITH SHOVEL. WALL STARTED COMING IN OPERATOR IMMEDIATELY WENT INTO TRAVEL POSITION REVERSE WALL MATERIAL ENGULFED CAB KILLING OPERATOR
