CONTRACTOR CONTRACTED TO DISASSEMBLE AND MOVE PORTABLE CONVEYORS. WHILE CONVEYOR SECTIONS WERE BEING LOADED ON SUBCONTRACTORS TRUCK. A SECTION DROPPED CRUSHING THE SUBCONTRACTERS TRUCK DRIVER. THE MINE WAS CLOSED FOR THE CHRISTMAS HOLIDAYS BUT THE CONTRACTOR WAS DOING A JOB HE WAS CONTRACTED TO DO.

THE BOLTS HOLDING THE CLAMP TOGETHER PULLED THROUGH THE BELT LET TING THE BELT ROLL BACK DOWN THE FRAME. HE WAS ON THE BELT AS IT ROLLED DOWN THE FRAME FOR APPROX. 250' AND THEN FELL APPROX. 30' TO THE BOTTOM OF THE PRIMARY HOLE.
