OPRATOR STARTED PINNING A CUT THAT THE MINER HAD ALREADY TAKEN OUT. HE STARTED ON THE 2ND BOLT AND THE INTERESECTION FELL AND PULLED OUT THE PERMANENT SUPPORT OVER TOP AND BEHIND THE PINNER TR APING EMPLOYEE. THE ROCK FALL WAS 20' X 30' AND 7' THICK.
