SCOOP OPERATOR WAS CLEANING ROCK THAT HAD BEEN SHOT DOWN FOR A BOLT CHANNEL,AND WENT OUT FROM UNDER PERMANENT ROOF SUPPORT.A PIECE OF ROCK APPROXIMATELY 14" THICK 4 FT. WIDE 6 FT. LONG FELL AN D CRUSHED HIM AGAINST SCOOP FATALLY INJURING HIM.
