THE EMPLOYEE WAS OPERATING A OFF STANDARD (LEFT DRIVE) SHUTTLE CAR, CAME INTO LOADING AREA, ALONG SIDE A COAL PILLAR. THE WALL OF THE PILLAR (RIB) FELL, COVERING THE VICTIM WITH COAL AND ROCK MATERIAL, CAUSING FATAL HEAD INJURIES.
