EMPLOYEE DRILLED INTO A MISFIRE, THAT HAD GONE UNDETECTED FROM THE DAY BEFORE. WHEN HE HAMMERED A WEDGE INTO THE STONE AN APPARENT SPARK IGNITED THE MISFIRE. THE EMPLOYEE WAS FATALLY INJURED.
