EMPLOYEE WAS ATTEMPTING TO SPLICE THE F-2 CONVEYOR BELT TOGETHER. HE CLIMBED UP ON THE BELT TO SECURE IT TO THE CONVEYOR FRAME. THE BOLTS HOLDING THE CLAMP TOGETHER PULLED THROUGH THE BELT LET TING THE BELT ROLL BACK DOWN THE FRAME. HE WAS ON THE BELT AS IT ROLLED DOWN THE FRAME FOR APPROX. 250' AND THEN FELL APPROX. 30' TO THE BOTTOM OF THE PRIMARY HOLE.
