Investigator narrative
EMPLOYEE'S WERE TRYING TO PULL TAIL SECTION BACK INTO PLACE. A PIPE HAD BEEN PUT ON HANDLE OF CHAIN FALL FOR EXTRA LEVERAGE. THE FALL CHAIN BROKE AND PIPE STRUCK INJURED IN CHEST. AS A RESU LT OF THE INJURY HE DIED AT HOSPITAL.
