I NAME OPERATING SCOOP IN REMOTE MODE SCOOPED FACE 3 ENTRY WHILE BACKING HE WAS STRUCK BY SCOOP CRUSHED BETWEEN SCOOP AND ROOF BOLTER SETTING IN X CUT CRUSHING BLOW CAUSED FATAL INJURIES TO RI GHT SIDE OF TORCO.
