THE REMOTE CONTROL CONTINUOUS MINER WAS BEING OPERATED IN THE FACE OF #9 ROOM OF THE 0 SOUTH OFF 1ST EAST UNIT#002-0. AT APPROX. 6:25 PM A MASSIVE ROOF FALL OCCURRED FATALLY CRUSHING THE MINER OPERATOR DANZIL GOINS. THE ROOF FALL WAS APPROX. 50 FT LONG, 14 FT WIDE, 0" TO 4FT THICK.
