INDIV WAS ATTEMPTING TO FREE A BLOCKAGE OF COAL AT OUT COAL SURGE PILE. INDIV DID NOT USE THE PRY BAR PROVIDED FOR THIS PURPOSE, NOR DID HE USE THE SAFETY LINE AND BELT WHICH IS PROVIDED. APPA RENTLY THE INDIV WALKED OUT (OVER) THE SURGE PILE FEED OPENING, TRIED TO DISLODGE THE BLOCKAGE BY KICKING AT IT. THE SIDES OF THE PILE GAVE WAY, AND THE MAN FELL INTO THE FEEDER SYNTRONS AND
