BASED ON TESTIMONY, WELDING WAS TO BE PERFORMED ON THE STAGELOADER AND PROBLEMS WERE EXPERIENCED SETTING THE WELDER UP. ACCORDING TO WITNESSES, WITH THE INCOMING POWER STILL ON, THE TOP PANEL WAS REMOVED AND THE EMPLOYEE REACHED INTO THE MACHINE, APPARENTLY ELECTROCUTING HIMSELF.
