THE EMPLOYEE WAS HAULING ROCK AT THE SURFACE ROCK BIN BUT IT WAS NOT YET READY TO LOAD HE PULLED PAST THE BIN UNTIL IT WAS READY TO BE DUMPED HE THEN BACKED BACK UNDER THE BIN THE BIN FELL CRU SHING THE CAB OF THE ROCK TRUCK CAUSING DEATH TOTHE EMPLOYEE NOTE ADDITIONAL INFORMATION WILL BEFORWARDED
