on 5/15/2026 miner was struck by loose rock from the back while kneeling to load lifters on the sill. Miner refused medical evaluation and completed shift and rotation. On 6/2/2026 miner stated EE had ache in right side pelvis area and was taken for medical evaluation and was placed on restriction for a contusion to the pelvis.

Employee was operating Jackleg when the gooseneck came apart and struck the employees left elbow resulting in a contusion.

Miner was aligning steel frame for air door installation. Miner asked for a spud wrench to align the top and bottom steel frames. During this time the steel shifted and the tip on the Miners left index finger was pinched in the hole resulting in a crushing injury with lacerations.

While training a miner using a jackleg with a 4' steel to drill a bolt hole in the back, EE was advised to reposition the leg. While repositioning the leg, the miner gave the cob too much pressure losing control of the drill. The drill fell backward towards the trainer hitting EE in the face splitting EE's lip. *The miner received 3 stitches by the medical facility's doctor.

I tripped over the fork of a Telle Handler while flying a drone.
Miner was installing a bearing set beam in the shaft at the 1-5 level, miner was using a spud wrench to align the beam when the wrench slipped out of the beam and caught a fingertip between the beam and wrench.

While hoist was being inspected and ran thru its pre shift checks it had a brake failure and lost the sinking bucket 100 to 150 feet down the shaft with the hoisting cable.
While drilling a face round with a jack leg drill, miner drilled into a vertical exploration hole from the surface, the hole was full of water and the pressure of 1300' of water forced the drill back into the miner chipping tooth.

Miner was unloading materials onto work deck in shaft from conveyance. Two beams were lying on the deck, miner was straddling the beams. When EE unhooked the rigging, the beams tipped and hit the miners foot.

While setting steel, a 10' piece of 3/8" angle iron fell three feet vertically onto the miners right foot. The steel punctured the miners right boot and also the skin on his foot. His status is Full Restriction, stay off feet and keep foot elevated. Possible small fracture and infection.
