HEAT - THE EMPLOYEE WAS ONLY OUT OF THE VEHICLE FOR 15 - 20 MINUTES FELT HOT. INCIDENT IS UNDER INVESTIGATION UNTIL MEDICAL REPORTS ARE OBTAINED TO COMPLETE OUR INVESTIGATION.
EMPLOYEE TRIP AND FELL ON LEVEL AREA.

Employees were positioning conveyor to load onto a flatbed trailer. EE walked between the two pieces of equipment when the conveyor shifted position at crest, striking him.

Changing screens. Employee was removing screens and walked forward. His lanyard got caught on one of the screens. Employee stepped back lanyard became loose employee pulled forward and fell forward on his hands.
Working in tight area ee states he strained/sprained muscles while sliding out. Employee continued working October 5 and October 6. Employee reported injury 10/7/05
EMPLOYEE DRANK TOO MUCH WATER, FLUSHING HIS SYSTEM. MEDICAL ATTENTION AT CASA GRANDE REGIONAL MEDICAL CENTER.

EMPLOYEE WAS CLEANING SAND OFF STACKER W/SHOVEL WHILE STANDING ON A TIRE ADHERED W/SAND. SHOVELING & STANDING ON TIRE CONTRIBUTED TO EE LOOSING HIS BALANCE AND FALLING.
