CONVEYOR STOPPED ALLOWING TAIL PULLEY TO GET BURIED BY OVER FLOW MATERIAL. THE EMPLOYEE LOCK OUT THE PANEL, REMOVED THE GUARD TO ACCESS AREA AND PROCEEDED TO CLEAR MATERIAL. ENOUGH MATERIAL WAS CLEARED OUT WHICH ALLOWED THE BELT TO SHIFT CAUSING HIS ARM TO GET PULLED INTO TAIL PULLEY.
