Employee was clearing rock from crusher when they felt a tweak in their shoulder. It was initially treated with first aid. When the condition didn't improve, the employee received an MRI. The event became reportable on 2/9/2022 when a doctor placed EE on total disability following the results of the MRI.

EE was removing nuts from the secondary crusher frame with a 1" pneumatic impact wrench. When the nut broke free the rotational force of the pneumatic wrench spun the nut up the threads pinning EE's right index finger between the handle on the impact wrench and the overhang of the crusher frame. EE received 7 sutures to the outside of the right index finger.

Injured employee examined what needed to be cleaned on the transfer screen to move the gates. EE was in a squatting position and barred the material that was stopping the gates from moving. In the squatting position EE grabbed the chute opening with hands out and when EE pulled self up EE felt a sharp pain in back and a popping sound.

An employee was hauling material to the dump grizzly, when they felt a pain in the lower back. The initial diagnosis was a strain to the lower back and they were returned to regular duties on 8/16/2014. No medical treatment was given at this time. The employee was further diagnosed and was removed from the duties on 6/9/2015.

The employee was pumping a manual grease gun when he felt some pain in the right elbow. The employee was sent to the clinic and was diagnosed with a minor strain to the elbow. The employee was sent back to regular duty with no retractions. The modifications were given based on the California Workers Comp. criteria only.

Employee was climbing down into the feeder that was being installed into #2 conveyor tunnel. The 4' "A" frame ladder slipped from under him and he hit his right elbow and right shoulder. His left knee was reported as sore due to walking on even surfaces during this project.

Employees welding lagging onto a tail pulley, injured employee said he caught a couple of flashes during the 11 hour shift. At the end of the shift he realized that he had received a flash burn in both eyes.

Two employees were replacing a bearing on the ecentric shaft of the Noble feeder in #2 conveyor tunnel. The shaft and bearings were supported by a chain and a chain hoist. One of the chains slipped allowing the bearing to twist and drop two inches, hitting employees left thumb and breaking it.
