Two miners were installing a section of metal pipe on a pontoon for the dredge using a Caterpillar TL943C forklift. The pipe shifted out of the pontoon cradle and struck the injured miner on the lower left leg causing a fracture.

Uneven ground, smooth rock. Poor footing. Worker slipped on packing material from metal the worker was transferring to pallet. Packing material covered a rock and worker fell and hurt left knee.

Employee hooked a chain to a jaw crusher cheek plate to be lifted by an operator using an excavator. The lifting chain's eye slipped, hitting employee in the mouth. Split right side of employee's lower lip, damaging four teeth.
