The employee was changing out a screen from a manbasket, when they tried to remove the screen over a bar, which caused a strain to the left shoulder.

The employee was dismounting a CAT 980F front end loader on level ground with engine off and parking break set. It was daylight the weather clear dry. While turning to face the ladder to dismount the employee twisted his right knee. The injury was diagnosed as a mild sprain. The injury became a time lost accident on 10/8/10 when surgery was required for a torn meniscus.

Ee was cleaning up around conveyor belt using a shovel & 1 1/2" water hose. When he returned to his ladder he felt a pain in his left shoulder (strain/sprain). This was initially under investigation by ins. co. for possible connection to prior injury - insurance company accepted claim as a new claim 10-14-04.
