THE EMPLOYEE ENTERED A LOG WASHER AND DID NOT FOLLOW SAFETY PROCEDURES BY USING A LOCK OUT TAG OUT PROCEDURE. THE MACHINE WAS INADVERTENTLY ENERGIZED WHICH IN TURN RESULTED IN HIS DEATH.

AVAILABLE INFORMATION FROM THE SCRAPER OPERATOR AND EYE WITNESS SHOWS THAT THE EMPLOYEE MOVED BETWEEN A BACKING 633 CAT SCRAPER AND HIS PICKUP OUT OF THE OPERATORS FIELD OF VISION. HE WAS RUN OVER BY THE LEFT REAR SCRAPER TIRE RESULTING IN DEATH.
