WHILE 2 EES WERE LOADING BOREHOLE, 1 CARTRIDGE OF EXPLOSIVE BECAME LODGED. WHILE TRYNG TO DEAL WITH THE LOADGED CARTRIDGE, IT DETONATED. THE DETONATION CAUSED SIGNIFICANT BODILY INJURY TO TWO EES. THE FORCE OF THE DETONATION CAUSED ONE EE TO GO OVER THE HIGHWALL; THIS EE DIED.

Employee was kneeling on a conveyor belt in order to access a plug in a feeder box. While unplugging the box, the conveyor belt unexpectedly started. The employee was pulled down the belt and their right foot got stuck on a piece of steel and caused their right ankle to break.

Lifting tarp that was covered in snow in order to cover tires. While lifting tarp, they felt a slight pull in lower back.

Miner was repairing a belt on the main feed. They attempted to get onto the belt and caught their left knee on a belt clip that was not secured down. It caused a laceration to their left knee. Miner was sent to the urgent care and was given five stitches. Miner was put on restricted duty but did not lose any work time.

Employee was using a cutting torch to cut a piece of steel. While cutting a piece of slag fell onto pant leg and burned through the pants. When the slag hit skin it caused a burn to left leg/shin.

Employee attempted to attach the welding trailer to the quarry maintenance truck but flipped the jack up and as he guided the trailer to the hitch the trailer dropped onto the top of his right foot. Employee was not using the supplied jack as he should have.

Injured was walking in the pit area, stepped over a fallen tree branch, foot landed on a rock that then split in two, causing him to lose footing and fall to the ground onto his left shoulder causing a left shoulder injury.

WHILE ATTEMPTING TO EMPTY CAN INTO DUMPSTER EMPLOYEE SLIPPED AND CAUGHT HIS LEFT PALM ON A PIECEOF METAL THAT WAS PROTRUDING FOMR THE SIDE OF THE DUMPSTER ITSELF. HE WENT TO THE CLINIC WHERE H IS HAND WAS STITCHED AND THEN RETURNED TO FULL DUTY IMMEDIATELY AFTER.

EMP STARTED DOWN THE HAUL ROAD WITH A LOAD OF STONE FOR THE PRIMARY CRUSHER WHEN FOR STILL UNKNOWN REASON TRUCK WENT OUT OF CONTROL,HIT A BERM ON LEFT & TURNED OVER ONTO THE TRUCK'S RIGHT SIDE . THE TRUCK REMAINED IN THE HAUL ROAD. EMP WAS REMOVED FROM THE CAB BY EMERGENCY PERSONNEL & TAKEN TO LOCAL ER. RELEASED IN 2 HRS.
