EE was attempting to turn a rotor on the Hazemag Impact Crusher when EE's right little finger became pinched between the blow bar and the inside liner of the crusher. Stitches were required and a fracture was also present.

EE was wiping face when they felt something in their eye. EE flushed the eye and that did make it better. EE was taken to an eye specialist and the foreign matter was removed from the eye with a mechincal probe and antiobitic eye drops were administered one time.

Employee was on back inspecting the inside of the sizing screen. As employee was crawling on back employee rested back on a dam causing employee pain to back. Employee was taken to clinic on 6/29/2018 and returned to full duty. On a follow up visit to the clinic on 7/12/2018 employee was put on restricted duty.

Employee was operating a haul truck and the bed of the truck was lifted by loader when the bucket pushed down material at the end of the bed. Bed lifted and came down shaking the truck.
EE was dismounting the 4600 Drag Line. When EE stepped on the second step from the top EE's foot slipped off. This caused EE to loose EE's handhold and fall to the ground. This resulted in a fracture to the LEFT hip. EE fell approximately 4'.

On Saturday October 4th, employee was welding a pipe from the trammel screen pan to our fines pump. Employee did not report any issues. Monday morning employee complaint about a sore back.
Employee informed that he felt "a little stiff" in the morning. He did not attribute it to any particular event at this time. We logged the incident in the first aid register at that time and followed up with him later that Friday. However on Tuesday morning 07/22/2014 he complained again of stiffness in his back. He was taken to a clinic. He was diagnosed with a strain back.

Employee complained about back pain on Wednesday 12/11/2013. He claimed that he felt OK through the weekend and Tuesday, but on Wednesday he reported some discomfort. He never reported this but until Wednesday 12/11/2013

A non-chargable incident occured at the end of the shift when the two dragline operators turned down the machine. One of the operators told his partner he believed he was having a heart attack. His partner called the EMS 911 and took him to the main gate. Five minutes later the ambulance arrived and took the employee to the hospital where he passed away about one hour later.

Two EE's were placing 6" PVC pipe at a tire wash. After gluing into place, they were back filling a Bobcat. As EE swung the backhoe for more material, injured EE reached down and pulled up on the pipe, the bucket of the backhoe passed, caught the tip of EE's right index finger between it and the end of the PV pipe, cutting a piece of skin.

The Counterweight did not line up with the saddle, a come-along was attached to the frame on the conveyor. He took a closer look to see what it was hanging up on and in the process put his hand on the frame of the take-up system, the weight shifted catching the tip of the middle and ring finger on his right hand between the frame and the weight.
While working on removing bucket pin on a 992 loader, dust went into employee's eyes, possibly from the exhaust from the Impact wrench while working in a confined area (employee was wearing prescription safety glasses with side shields).
Employee was splicing the wire for the installation of a replacement light fixture on the #2 Hazemag crusher, as he was cutting off the outer cover of the cable the knife slipped and cut his thumb.

Employee was washing down under the secondary plant screening tower with a water hose, when he lost his footing on the ground and fell.
Employee slipped off the last rung of a ladder and bumped his right knee on the catwalk surface of a conveyor on 2-16-05: (Employee requested medical attention for the first aid incident on 2/23/05, at which point he was taken to workers compensation.)
Employee was changing a broken blow bar in the Humbolt Primary crusher and a small sliver of the blow bar struck him in the left leg.
EE was attempting to tighten a bolt on the drag friction with a hand wrench. The wrench was struck by a drum bolt causing EE's ring finger on L. hand to become pinched between the wrench and drag friction. A fracture to the last digit of the ring finger on the L. hand was realized.

Employee bumped his knee on the machine, while making repairs on a 926 loader. NOTE: (EE sought medical treatment on January 17th, 2005; prior to this date, it was a first aid log entry)
Employee was removing a bearing sleeve from a shaft with a hammer and a shard of metal broke off and cut the employees left arm.
OPERATOR WAS RAISING UP THE BOOM. HE GRABBED THE MASS PIPE AND THE BOOM STABILIZER CRUSHED HIS FINGERS.
EE WAS LAYING OUT BOOSTERS ON SHOT & GROUND WAS MUDDY. EE SLIPPED AND STRAINED HIS BACK WHILE TRYING NOT TO FALL.
THE EE WAS EXITING HER PICKUP AT THE 4600 DRAGLINE WHERE SHE WORKS, WHEN SHE WENT TO CLOSE THE DOOR, SHE REALIZED THE SEATBELT IN THE WAY AND WENT TO GRAB IT, WHICH CAUSED HER TO CATCH HER RIGHT HAND IN THE DOOR AS IT CLOSED.
Employee was welding inside the welding shop and his foot became entangled in the welding lead which caused him to fall and hit his shoulder on the floor. The ee did not seek medical treatment for the injury until Nov 24, 2004.

EE TOOK OFF SAFETY GOGGLES TO CLEAN OFF DUST. WHEN HE FLEW INTO HIS GOGGLES A FOREIGN BODY FLEW INTO HIS RIGHT EYE. '
