7 recorded(Under investigation) (Per Supervisor) Employee was assigned task of pushing overburden wall down with bulldozer. This was being done to install a water jack (floating pump) to pump out old mine cut. A short period of time later bulldozer and employee were discovered under water in mine cut by another employee.
EMPLOYEE STATES HE WAS USING BAND SAW TO CUT NOTCH IN WOOD AND PLACED HIS HAND UNDER BLADE CAUSING SEVERE CUT TO HAND. (LT).
DECEASED FAILED TO SECURE ELECTRICAL POWER SOURCE RESULTING IN ELECTROCUTION.
EMPLLOYEE IN 60' HIGH REACH GETTING TOOLS DOWN FROM JOB ON C-330 BELT, WHEN A GUST OF WIND TURNED THE HIGH-REACH OVER WITH EMPLOYEE IN MAN BASKET.
UNDETERMINED CAUSE OF ACCIDENT. DRIVER DROVE VEHICLE INTO DITCH.
