Struck by... (Not Elsewhere Classified)

Employee was using a piece of rebar in an attempt to dislodge a rock that was wedged between the conveyor frame and the head pulley of the VSI crusher feed conveyor. In doing this, the rebar got between the head pulley and the belt while it was running. This caused the rebar to strike both of EE's hands resulting in the amputation of the left thumb and the tip of the right thumb.

Fall to the walkway or working surface

Office employee fainted, due to an ongoing personal medical issue, and fell striking right shoulder on the ground. This resulted in a dislocated shoulder and small fracture to one of the bones in the shoulder.

Caught in, under or between a moving and a stationary object

(IW) was adjusting the crusher apron with shims on the portable crushing unit. A hydraulic ram that raises the aprons had a bent cylinder, which limited the stroke. The preshift inspection sheet did not indicate an issue nor was it reported to the Manager. IW installed a makeshift support whereas the tip of EE's thumb caught between the cylinder and the makeshift support

Fall from machine

The employee lost footing on the last rung of the ladder while dismounting dragline, and fell to the ground. Recommendation: utilize a ridged mounted flat bar step in lieu of swinging chain and bar stock. Re instruct employee in safely mounting and dismounting equipment utilizing the 3 point method.

Employee was removing the plastic coating from the hoist cable with a utility knife. The knife was being drawn towards employee instead of away from body.

Contact with hot objects or substances

EE was pressure cleaning with hot water a discharge head pulley on the VSI. His footing was lost due to grease from the cleaning process the trigger was engaged on the pressure washer and the spray from the nozzle struck his ankle.

Employee was cutting a wiper for the skirt board on the crusher. The blade on the utility knife broke off and lacerated hand.

Accident type, without injuries

Conditions: Sunny, stable ground. The fully loaded tractor trailer with broken concrete was discharging load when a portion of the load was lodged at the driver side front of the trailer. The offset load caused the trailer to overturn towards the drivers side. The overturn resulted in damage to the tractor trucks drivers side of cab and twisted frame. Operator was not injured.

Employee was lifting an approximately 40 pound bucket of rock from a pickup bed. The employee strained his upper back and shoulders while lifting this bucket of rock.

EE was operating a Skid Steer Loader on 9/21/11 when he started having severe chest pains. he was immediately taken to hospital by his supervisor. The next morning the EE told his super.that during a med. exam it was determined that the chest pains were caused by a severe case of scoliosis,and was not work related. The EE was separated from SDI Quarry, Inc. on 9/22/11.

Employee was installing bucket teeth on a front end loader using a hammer. Upon impact of hammer with bucket teeth two metal fragments dislodged and struck employee in the right shoulder and the left chest pectoral muscle.

Struck by falling object

Employee was carrying an air impact wrench with a three inch socket attached. The socket fell off the wrench striking the employees right foot.

Caught in, under or between a moving and a stationary object

Employee was assisting with the re-installation of a skirt board bracket. While attempting to reinstall the skirt board bracket, the bracket slipped, pinching the employee's right index finger.

EE was removing a skirt board rubber mounting plate from a conveyor. The mounting plate slipped pinching the right thumb of the ee, removing a section of skin from the front of the ee right thumb.

Struck against a moving object

EE was driving an operational F-350 Ford truck, when he veered off the road, was over 200' wide, running into a pile of boulders. EE claims that he experienved transmissino gear problems causing him to veer off road. The EE was given a post accident exam by a doctor who could not find any medical reasons to caues incident. EE denies he lost contrl by distraction or falling asleep.

Absorption of radiations, caustics, toxic and noxious substances

Employee first reported on 11/8/06 that he had spilled mineral spirits on his left foot on 10/23/06 at approx. 3:00 p.m. EE's foot became infected approx. 11 days later, requiring hospitalization. EE was using mineral spirits to clean grease from 4600 dragline.
